You can make a referral by completing our online referral form or by contacting our team via
phone or email.
Who can refer to Healthproof’s services?
Referrals can be made by healthcare professionals, support coordinators, aged care service
coordinators, case managers, family members, or the clients themselves.
What information is required for a referral?
We will need basic client contact details, including current health concerns, and any relevant
funding information (NDIS, HCP, etc.).
Is there a waiting period after submitting a referral?
Once a referral is submitted, we typically review and respond within 24-48 hours to discuss next
steps.
Can I refer a client for multiple services at once?
Yes, you can refer a client for multiple allied health services, such as physiotherapy, occupational
therapy, speech pathology, podiatry, psychology or dietetics, within the same referral.
What areas does Healthproof provide services in?
We offer face-to-face services in Sydney and Melbourne, as well as Telehealth services across
Australia.
How do I know if my referral has been received?
You will receive a confirmation email or phone call once we have successfully received and
reviewed the referral.
What funding options are accepted for referrals?
We accept NDIS, Home Care Package (HCP), Commonwealth Home Support Program (CHSP), Workers Compensation and CTP, Private Health Insurance, Medicare EPC and private funding.
How soon can a therapist see the client after a referral?
Appointment availability depends on the therapist’s schedule, but we aim to arrange initial appointments within one week.
Can I track the progress of my referral?
Yes, our team will keep you updated throughout the referral process and inform you of any relevant milestones, including appointment scheduling.
Do I need a doctor's referral to access Healthproof's services?
No, a doctor’s referral is not required to access our services; however, we do accept referrals from GPs, specialists, family members, and other healthcare providers.
Can family members or carers make a referral on behalf of a client?
Yes, family members or carers can make a referral with the client’s consent to help initiate the care process.
Do you accept NDIS participants and government-funded packages?
Yes, we accept NDIS participants, as well as clients with Home Care Packages (HCP) and Commonwealth Home Support Packages (CHSP).

Our team will contact you to discuss the suitability of our services and our therapists will conduct an initial assessment to identify the client’s needs and recommend the most suitable services.

Absolutely, we offer Telehealth services nationwide, and clients can be referred for virtual consultations and therapy sessions.
