A dietitian considers health conditions, food intake, appetite, medications, access to food, culture, preferences, cooking facilities, support needs and the person’s goals. Recommendations are individual and should be coordinated with medical and allied health care where required.
Personalised Nutrition Support at Home
Nutrition advice is more useful when it reflects what a person can realistically buy, prepare, eat and maintain. A home visit can help the dietitian understand the practical context around meals rather than giving a generic meal plan that does not fit the person’s circumstances.
The dietitian may consider:
- usual meals, snacks and fluids;
- appetite and recent changes in intake;
- food preferences, culture, beliefs and sensory needs;
- allergies, intolerances or medically required restrictions;
- shopping, budget and food availability;
- cooking skills, kitchen access and support from others;
- chewing, swallowing or mealtime concerns that require speech pathology;
- medications, supplements and relevant pathology or medical information;
- weight history and changes where clinically relevant; and
- the person’s priorities and readiness for change.
The aim is not to judge food choices or impose unnecessary restrictions. The dietitian will explain which recommendations matter most, what evidence supports them and how they can be adapted to your routine.

A dietitian may support meal patterns, carbohydrate awareness and sustainable dietary changes for diabetes or other metabolic concerns, in coordination with the person’s medical team. Medication changes and glucose-management decisions remain with the appropriate prescriber.

Nutrition advice may address dietary patterns, sodium, fats, fibre and other factors relevant to cardiovascular risk. Recommendations should reflect diagnoses, medications, kidney function, appetite and the person’s overall care plan.

Restrictive diets are not promoted without assessment, a clear purpose and a plan to review or reintroduce foods where appropriate. NDIS Dietitian Services in Sydney
NDIS dietetics may be considered when nutrition support is related to the participant’s disability and functional goals, rather than for a general health or lifestyle purpose alone.
Depending on need and scope, a dietitian may assist with:
- assessing nutrition adequacy;
- practical meal planning and food access;
- disability-related eating routines or sensory considerations;
- education for the participant and support network;
- nutrition strategies that support health and daily participation;
- coordination with speech pathology where swallowing or texture is relevant;
- written recommendations or reports where clinically required; and
- review of progress and implementation.
Confirm your funding pathway, plan management and any report requirements with our team before your first appointment. A dietitian’s recommendation does not guarantee NDIS funding for food, supplements, meal preparation, equipment or ongoing appointments.
Nutrition Support for Older People
Nutrition needs can change with age, illness, hospitalisation, reduced mobility and social circumstances. A home dietitian appointment may be useful when the person’s actual kitchen, food supply, meal support and appetite need to be understood.
The assessment may explore:
- recent weight or appetite changes;
- meal frequency and portion tolerance;
- protein and energy intake;
- hydration;
- chewing, dentition or swallowing concerns;
- ability to shop, prepare and store food;
- support from family, carers or meal services;
- supplements already being used;
- constipation or other gastrointestinal issues; and
- relevant health conditions and medications.
Recommendations might involve enriching familiar foods, changing meal timing, simplifying preparation, improving access or discussing a suitable supplement. Supplements are not a universal replacement for food or medical assessment.
Dietitian and Speech Pathology Roles at Mealtimes
Dietitians and speech pathologists may both be involved when eating or drinking is difficult, but their roles are different.
A speech pathologist assesses swallowing, communication and mealtime safety. A dietitian assesses nutritional adequacy, dietary intake, hydration and whether the recommended food or fluid pattern meets the person’s needs.
Where a texture-modified diet or thickened fluids are recommended, the dietitian can help consider nutritional consequences and practical implementation. The dietitian does not independently diagnose swallowing safety, and the speech pathologist does not replace a full nutrition assessment.
What Happens During a Home Dietitian Appointment
Before the Appointment
Provide the referral reason, relevant diagnoses, medication and supplement list, recent medical or pathology information where available, allergies, swallowing recommendations and current funding details. A food record can be useful but is never mandatory.
Nutrition Assessment
The dietitian discusses health, food and fluid intake, appetite, symptoms, routines, preferences, access and goals. Measurements may be considered where clinically useful and consented to, but an appointment is never reduced to weight alone.
Practical Recommendations
The plan may include meal structure, food choices, fortification, hydration, shopping or preparation strategies, symptom-related advice, education or coordination with other clinicians. Recommendations are prioritised so you know what to do first.
Written Plan and Follow-Up
Where appropriate, the dietitian can provide a written summary in an accessible format. Follow-up can review what was practical, what changed and whether the plan needs adjustment.
Collaboration
With consent, the dietitian may communicate with the GP, specialist, speech pathologist, occupational therapist, support coordinator, family or other relevant people. Information is shared only for a clear care purpose.
Dietitian or Nutritionist – What Is the Difference?
In Australia, the terms dietitian and nutritionist are not interchangeable.
An Accredited Practising Dietitian, or APD, has completed recognised dietetics education and participates in Dietitians Australia’s credentialling program. Dietitians can provide medical nutrition therapy within their competence as well as broader nutrition advice.
The term nutritionist can be used by people with different levels of training, so it is worth asking about a clinician’s specific qualifications.
Referrals and Funding Pathways
A direct private enquiry may be possible. Other pathways can include NDIS, Medicare under an eligible referral, private health, aged-care, DVA or compensable arrangements, depending on the person, clinician and current program rules.
Before confirming a funded appointment, we will check:

- whether the clinician can work under the intended program;
- referral and eligibility documentation;
- the approved appointment type and duration;
- travel and report requirements;
- billing and claiming arrangements; and
- any gap or out-of-pocket cost.
