Speech Pathology
Speech pathology may assist people with communication, speech, language, fluency, voice, swallowing or mealtime concerns, depending on the clinician’s scope and current service availability. Assessment in the person’s usual environment can help identify practical communication demands and the support available around them. Podiatry
Mobile podiatry focuses on foot and lower-limb health. Depending on assessment and service scope, support may include nail and skin care, foot pain, footwear advice, diabetic foot monitoring, gait concerns and discussion of orthotic options. Dietetics

Who We Support
Healthproof’s mobile model is designed for people whose needs are better understood or supported in a home, school, work or community setting. Service suitability and clinician availability are confirmed individually.
NDIS Participants and Support Networks
Healthproof can receive enquiries and referrals from NDIS participants, nominees, families, support coordinators, plan managers and other professionals. Assessment and therapy are linked to the person’s functional goals and funded support needs. NDIS funding is not automatic; the service, plan, management type and available budget are confirmed before appointments proceed.
Older People Living at Home
Older people may seek support with mobility, balance, foot health, nutrition, communication, safe daily activities or maintaining participation at home and in the community. Healthproof can discuss the most relevant discipline and whether a private, aged-care or other referral pathway may apply.
People Recovering From Injury, Surgery or Illness
A person may benefit from home-based support when travel is difficult or when rehabilitation needs to be practised in the environment where daily activities occur. The appropriate service could include physiotherapy, occupational therapy, dietetics, podiatry or exercise physiology, depending on clinical assessment.
People Living With Chronic or Complex Health Needs
Long-term conditions can affect movement, fatigue, nutrition, communication, daily routines and confidence with activity. One discipline may be sufficient, or coordinated input from more than one clinician may be useful. Each service remains separately assessed, planned and booked.
Families, Carers and Referring Professionals
With the person’s consent, clinicians may involve family members, carers, support workers and relevant professionals in goal setting, education or practical implementation. Our referral pathway lets the referrer identify the required discipline and provide information that helps the team triage the enquiry.

You do not need to diagnose the exact treatment required before contacting Healthproof. The referral or enquiry should describe the person’s main concerns, goals, location, preferred appointment setting, funding pathway and any safety or communication requirements.

Registered NDIS Provider for Six Allied Health Services
Healthproof Pty Ltd is listed by the NDIS Quality and Safeguards Commission as an approved registered provider. Healthproof provides six allied health disciplines: physiotherapy, occupational therapy, speech pathology, dietetics, podiatry and exercise physiology.
NDIS participants may be agency managed, plan managed or self managed. We confirm your management type, relevant plan details, service agreement requirements and available funding before the first appointment. Healthproof cannot guarantee that a requested service will be considered reasonable and necessary or funded by the NDIS.
Why a Home or Community Visit May Be Useful
Assessment in a Real Environment
A clinician can see the spaces, routines and practical barriers that influence function. This may produce more relevant observations than asking the person to describe every detail from memory.
Reduced Travel Burden
Home visits can reduce the physical and logistical demands associated with transport, parking, clinic access and waiting areas. This is particularly relevant for people with mobility limitations, fatigue, disability or caring responsibilities.
Practical Strategies Where They Will Be Used
Exercises, equipment, communication tools, daily-living strategies and meal-planning recommendations can be discussed in the setting where the person will apply them. Recommendations still depend on assessment, safety and the scope of the relevant clinician.
Involvement of the Right Support People
Where the person consents, a family member, carer, support worker or coordinator may join part of the appointment. Their involvement can support shared understanding without replacing the person’s own preferences and decisions.
Access to a Multidisciplinary Team
Healthproof offers six disciplines through one organisation. This can make referral and communication simpler when more than one service is genuinely required. It does not mean every person needs multidisciplinary care or that several clinicians can always attend at the same time.
How Mobile Allied Health Appointments Work
1. Enquiry or Referral
Contact Healthproof or submit the referral form with the person’s details, main concern, location, requested discipline and funding information.
2. Intake and Service Matching
The team reviews the enquiry, checks whether the requested service is appropriate for triage and confirms the information still needed. Clinical questions may need to be referred to a clinician.
3. Initial Assessment
The clinician discusses relevant history, current function, goals, risks and the person’s priorities. The assessment varies by discipline and may include observation, practical tasks or standardised measures where appropriate.
4. Plan and Ongoing Review
The clinician explains findings and discusses suitable recommendations. Ongoing appointments, home activities, reports, equipment or referrals are arranged only where clinically appropriate and agreed.
