Our Mobile Health Services

Six Allied Health Disciplines Across Greater Sydney

Our expert team offers in-home therapy and rehabilitation, addressing various health conditions with personalised treatment plans designed to promote better mobility, pain management, and overall well-being.

Physiotherapy

Mobile physiotherapy may support movement, strength, balance, mobility, pain management, rehabilitation and confidence with everyday physical tasks. A physiotherapist can assess how a condition, injury, surgery, disability or period of reduced activity is affecting function, then discuss an individual plan. See the physiotherapy page for home visits, NDIS physiotherapy, post-operative rehabilitation and falls risk.

Occupational Therapy

Occupational therapy focuses on the activities a person needs or wants to do in daily life. Mobile occupational therapy may include functional assessment, daily living strategies, assistive technology, equipment, home safety, routines and recommendations that support participation and independence. The occupational therapy page explains functional capacity assessments and NDIS occupational therapy in more detail.

Speech Pathology

Speech pathology may assist people with communication, speech, language, fluency, voice, swallowing or mealtime concerns, depending on the clinician’s scope and current service availability. Assessment in the person’s usual environment can help identify practical communication demands and the support available around them. The speech pathology page covers communication assessment, AAC and swallowing support.

Podiatry

Mobile podiatry focuses on foot and lower-limb health. Depending on assessment and service scope, support may include nail and skin care, foot pain, footwear advice, diabetic foot monitoring, gait concerns and discussion of orthotic options. Visit the podiatry page for service-specific information and signs that may require more urgent medical attention.

Dietetics

A dietitian can assess nutrition needs in the context of health conditions, appetite, food access, eating patterns, medications, culture, preferences and daily routines. Mobile dietitian appointments may support practical meal planning, nutrition adequacy and the management of clinically relevant dietary concerns. The dietitian page explains what happens during an appointment and how funding pathways may apply.

Exercise Physiology

Exercise physiology uses individualised clinical exercise to support health, function and participation. An Accredited Exercise Physiologist may work with people managing chronic conditions, disability, reduced strength or endurance, deconditioning, injury recovery or barriers to physical activity. The exercise physiology page explains how it differs from general fitness training and from physiotherapy.

Who We Support

Healthproof’s mobile model is designed for people whose needs are better understood or supported in a home, school, work or community setting. Service suitability and clinician availability are confirmed individually.

NDIS Participants and Support Networks

Healthproof can receive enquiries and referrals from NDIS participants, nominees, families, support coordinators, plan managers and other professionals. Assessment and therapy are linked to the person’s functional goals and funded support needs. NDIS funding is not automatic; the service, plan, management type and available budget are confirmed before appointments proceed.

Older People Living at Home

Older people may seek support with mobility, balance, foot health, nutrition, communication, safe daily activities or maintaining participation at home and in the community. Healthproof can discuss the most relevant discipline and whether a private, aged-care or other referral pathway may apply.

People Recovering From Injury, Surgery or Illness

A person may benefit from home-based support when travel is difficult or when rehabilitation needs to be practised in the environment where daily activities occur. The appropriate service could include physiotherapy, occupational therapy, dietetics, podiatry or exercise physiology, depending on clinical assessment.

People Living With Chronic or Complex Health Needs

Long-term conditions can affect movement, fatigue, nutrition, communication, daily routines and confidence with activity. One discipline may be sufficient, or coordinated input from more than one clinician may be useful. Each service remains separately assessed, planned and booked.

Families, Carers and Referring Professionals

With the person’s consent, clinicians may involve family members, carers, support workers and relevant professionals in goal setting, education or practical implementation. Our referral pathway lets the referrer identify the required discipline and provide information that helps the team triage the enquiry.

One Referral Pathway for Multiple Allied Health Services

You do not need to diagnose the exact treatment required before contacting Healthproof. The referral or enquiry should describe the person’s main concerns, goals, location, preferred appointment setting, funding pathway and any safety or communication requirements.

The team can then:

Review

We review the information provided.

Match

We confirm which discipline appears most relevant.

Check

We check service-area and clinician availability.

Clarify

We request additional clinical or funding information where needed.

Book

We arrange the first appointment, or explain another appropriate next step.

A referral does not guarantee acceptance, a specific clinician, a particular appointment time or funding approval. Urgent and emergency health concerns are not managed through the website referral form.

NDIS Registered Provider

As a registered NDIS provider, Healthproof brings extensive experience

At Healthproof, we’re proud NDIS providers with lots of experience helping people with disabilities and complex needs. Our mission is to help improve the independence, mobility, well-being, quality of life and comfort of patients with disabilities by providing them access to quality healthcare services.

Allied Health Services for NDIS and Aged Care Coordinators

In addition to serving individual clients, we also work closely with support coordinators and service coordinators in the NDIS and aged care sectors. We understand the unique challenges faced by coordinators in sourcing appropriate services for their clients. That’s why we provide our mobile allied health services and support to help them find the right solutions that meet the needs of aged care residents and NDIS participants.

Registered NDIS Provider for Six Allied Health Services

Healthproof Pty Ltd is listed by the NDIS Quality and Safeguards Commission as an approved registered provider. Healthproof provides six allied health disciplines: physiotherapy, occupational therapy, speech pathology, dietetics, podiatry and exercise physiology.

NDIS participants may be agency managed, plan managed or self managed. We confirm your management type, relevant plan details, service agreement requirements and available funding before the first appointment. Healthproof cannot guarantee that a requested service will be considered reasonable and necessary or funded by the NDIS.

Why a Home or Community Visit May Be Useful

Assessment in a Real Environment

A clinician can see the spaces, routines and practical barriers that influence function. This may produce more relevant observations than asking the person to describe every detail from memory.

Reduced Travel Burden

Home visits can reduce the physical and logistical demands associated with transport, parking, clinic access and waiting areas. This is particularly relevant for people with mobility limitations, fatigue, disability or caring responsibilities.

Practical Strategies Where They Will Be Used

Exercises, equipment, communication tools, daily-living strategies and meal-planning recommendations can be discussed in the setting where the person will apply them. Recommendations still depend on assessment, safety and the scope of the relevant clinician.

Involvement of the Right Support People

Where the person consents, a family member, carer, support worker or coordinator may join part of the appointment. Their involvement can support shared understanding without replacing the person’s own preferences and decisions.

Access to a Multidisciplinary Team

Healthproof offers six disciplines through one organisation. This can make referral and communication simpler when more than one service is genuinely required. It does not mean every person needs multidisciplinary care or that several clinicians can always attend at the same time.

How Mobile Allied Health Appointments Work

HealthProof-Home-Service-449

1. Enquiry or Referral

Contact Healthproof or submit the referral form with the person’s details, main concern, location, requested discipline and funding information.

HealthProof-Home-Service-491

2. Intake and Service Matching

The team reviews the enquiry, checks whether the requested service is appropriate for triage and confirms the information still needed. Clinical questions may need to be referred to a clinician.

HealthProof-Home-Service-88

3. Initial Assessment

The clinician discusses relevant history, current function, goals, risks and the person’s priorities. The assessment varies by discipline and may include observation, practical tasks or standardised measures where appropriate.

HealthProof-Home-Service-90

4. Plan and Ongoing Review

The clinician explains findings and discusses suitable recommendations. Ongoing appointments, home activities, reports, equipment or referrals are arranged only where clinically appropriate and agreed.

Allied Health Care Built Around Clear Clinical Roles

Healthproof’s team includes clinicians from six allied health disciplines, and each clinician’s credentials are shown on their own profile rather than through a single blanket claim.

Physiotherapists, occupational therapists and podiatrists are regulated professions in Australia and hold current registration appropriate to their role. Speech pathologists, dietitians and exercise physiologists work under profession-specific credentialling arrangements. This gives patients and referrers a clear basis for understanding who they are working with.

Mobile Allied Health FAQs

What mobile allied health services does Healthproof provide?

Healthproof provides physiotherapy, occupational therapy, speech pathology, dietetics, podiatry and exercise physiology. The available discipline, clinician and appointment setting depend on your needs, location and our current capacity.
Healthproof provides services across Greater Sydney, but coverage can vary by discipline and postcode. Provide the service you need and the appointment address so our team can confirm availability.
Potentially. Healthproof provides appointments at homes, schools, workplaces, aged-care settings and community locations. The setting needs to be appropriate, private enough for the service and safe for both you and the clinician.
Telehealth may be available for selected services and clinical needs. It is not suitable for every assessment or treatment, and availability should be confirmed with the relevant discipline.
Some private or NDIS enquiries may be made directly, while Medicare, compensable, aged-care or other funded pathways may require specific documentation or approval. Confirm the service and intended funding pathway before booking.
Healthproof is an approved registered NDIS provider. The requested allied health service still needs to be relevant to the participant’s goals, plan and funding, and we need suitable clinical capacity available.
That depends on the discipline, clinician, referral, policy or program rules and the way the appointment is delivered. Confirm the billing pathway with our team before your appointment.
Yes, where separate assessments identify a genuine need. Each discipline has a distinct clinical role, and separate appointments, consent, goals and funding may be required.
Use the online referral form or contact our team. Include the person’s contact details, requested service, location, goals, relevant health information, communication or access needs and funding pathway.
This website and our routine mobile appointments are not emergency services. For severe breathing difficulty, chest pain, signs of stroke, uncontrolled bleeding, serious injury, choking or another immediate threat to life, call 000.
