Welcome to Healthproof, an NDIS registered mobile physiotherapy, podiatry, occupational therapy and speech pathology service provider in Australia. Please read these Terms of Service carefully before using our services. By accessing or using the service, you agree to be bound by these terms. If you disagree with any part of the terms, then you may not access or use the service.
- EligibilityTo use our services, you must be at least 18 years old or have the consent of a parent or guardian if you are younger than the age of 18. By using our Service, you represent and warrant that you meet these eligibility requirements.
- ServicesWe provide a range of services, including mobile physiotherapy, podiatry, occupational therapy, National Disability Insurance Scheme (NDIS) provision, and speech pathology services. Our services are available for individuals, families, and organisations and are provided by fully qualified professionals who will travel to your location to provide treatment.
- Appointments
- Booking: To schedule an appointment, you must provide us with accurate and complete information, including your name, contact details, and relevant medical history. You can book an appointment through our website, phone, or email.
- Cancellations and Rescheduling: If you need to cancel or reschedule an appointment, please provide at least 48 hours’ notice. Failure to provide sufficient notice may result in a cancellation fee of the full session amount charged to your account.
- Late Arrivals: If you are late for your appointment, your session may be shortened, and you will still be charged the full session fee.
- Payment and Fees
- Prices: Our current fees for services are available on our website and may be subject to change without notice. You are responsible for paying all fees associated with the use of our services.
- Payment Methods: We accept various payment methods, including cash, credit/debit cards, and direct deposit. Payment is due at the time of service unless otherwise agreed upon in writing. We may also accept payment through NDIS, DVA, HCP, CHSP, workers compensation and CTP, health insurance and Medicare.
- Health Insurance, Medicare Rebates, and NDIS Funding
- Health Insurance and Medicare: If you have private health insurance or are eligible for a Medicare rebate, it is your responsibility to ensure you have the appropriate coverage and to claim any rebates directly from your insurer or Medicare.
- NDIS Funding: If you are an NDIS participant, you are responsible for ensuring we have adequate information regarding the funding in your plan to cover the services provided by Healthproof. You must inform us of your NDIS status and provide us with the necessary information to process service bookings and claims.
- Client Responsibilities
- As a client, you agree to:
- Provide accurate and complete information about your health and medical history.
- Follow the treatment plan and recommendations provided by our therapists.
- Communicate any concerns, questions, or changes in your condition to your therapist.
- Treat our therapists and staff with respect and courtesy.
- Privacy and ConfidentialityWe are committed to protecting your privacy and maintaining the confidentiality of your personal and health information. Our Privacy Policy, available on our website, explains how we collect, use, and disclose your personal information.
- Intellectual PropertyOur website and services may contain intellectual property owned by us or third parties. You may not use our intellectual property without our prior written consent.
- Limitation of LiabilityTo the fullest extent permitted by law, Healthproof and its therapists, employees, and agents shall not be liable for any direct, indirect, incidental, special, consequential, or punitive damages, including but not limited to loss of profits, loss of data, or personal injury, arising out of or in connection with your use of our services.
- Governing LawThese terms shall be governed and construed in accordance with the laws of Australia, and any dispute arising out of or in connection with these Terms will be subject to the exclusive jurisdiction of the courts of Australia.
- Changes to TermsWe may modify these terms from time to time. Any changes will be effective immediately upon updating our website. Your continued use of our services after any changes to these terms constitutes your acceptance of the modified terms.
